Battery Charger - Overheating at the plug

I've seen that kind of thing before when arcing occurs inside the outlet (usually because there's a poor connection between the outlet and the accessory plug).

 

Certainly get the EVSE looked at (I'd also recommend replaced - chances are the plug was/is now damaged), but I'd suspect the outlet itself had a part to play in that as well - it possibly it had a poor connection internally. The car will only pull up to 12A from a 120V outlet - under no circumstances should a sustained 12A done that to a normal GFCI outlet.

Which charging cord do you have... the one with a white J1772 that plugs into the car, or a black J1772 plug?  The white ones were recalled for potential to overheat at the wall plug and not detect it.  The black ones have a thermistor inside the plug to sense oveheating.  If you have a white one, get it replaced.  There was a recall several years ago, but not sure if it would still net you a new cord for free.  Ask your dealer.
