My 2016 Fusion Energi is doing this sort of thing for the last few weeks. Outlet (120V) does not get hot. When I plug in the charge adapter, the charge ring will light up and go through the sequence most of the time, but sometimes not. Sometimes it will go through the sequence and will light up the first quadrant LED but I know the charge level is higher than 25%. If this happens then the first quadrant LED will go off after a minute. When it charges normally I can hear a series of relay clicks just after plugging in the adapter and if I don't hear that "Click, click-click" I know that it is not going to charge. I had it to the dealer and they reported no error codes and are going to research it.
I have tried unplugging, start the car, stop the car, and then try to charge again. Sometimes this works, mostly not.
